Health officials in Kentucky have traced a recent salmonella outbreak to a restaurant in Hopkins County.
The restaurant has been identified as Casa Mexicana.
During the outbreak, more than 12 people became sick with salmonella and one person died.
At least eight of the cases have been linked to that restaurant.
Four other cases have been reported in other counties.
Health officials who visited Casa Mexicana say they found numerous violations, which prompted them to test food samples and shut down the restaurant.
