NASHVILLE, TN (WSMV) -

The state cited a care home over the escape of a patient once committed to the Middle Tennessee Mental Health Institute.

In July, Kenneth Mallady left Anne's Care Home in Antioch. He was being treated there after being found not guilty by reason of insanity and discharged from the institute into outpatient care.

After his escape, police say he attacked a store clerk and had several bizarre run-ins with officers across middle Tennessee before being captured.

The state cited Anne's Care Home for failing to report his disappearance. The facility's owner, Kelvin Talley, responded by writing that he would report future incidents to the state, "as soon as possible."
