The mother of a ten-year-old boy said she wants justice after she said her son was injured at a controversial youth center.

Edith Ruland‘s son was a client of Chad Youth Encouragement Center until the day she saw his bruises and photographed them with a camera.

Ruland sent her son, Dennis, to the center for his behavior problems. A month later, she said he was covered with bruises.

"He showed me at least 20 (bruises) on one leg," said Edith Ruland.

She took pictures with her digital camera of bruises on her son’s arms and shins along with scratches to his neck and chin.

"I would be slammed down on the floor. I would be slammed up against the wall," said Dennis Ruland.

"And he was crying, shaking and saying, 'Momma, please don't make me stay here anymore,'" said Edith Ruland.

She said she never would have sent her son to Chad, which is a privately owned mental health facility, if she had been told that in 2005, a 14-year-old girl who was being escorted by the staff later died.

Then in June of 2007, a 17-year-old died after being restrained by the center’s staff.

A state investigation released on Friday shed new light on the 17-year-old's death. The report from the state department of health said the teen was put in a protective hold by a staff member, and he began to calm down.

Then, according to a nurse, he was unresponsive with no pulse, and he wasn't breathing.

A doctor at Vanderbilt Hospital found traumatic injuries including a bruised trachea, internal bleeding and he had extensive swelling and bleeding in the brain.

The official autopsy has not been released.

Edith Ruland said she witnessed one incident herself in which an elementary-age child complained of a stomach ache and left the lunch line. She said she saw two staffers throw him to the ground and put a knee in his back.

"This child by this time was yelling and screaming, 'You're hurting me. You're hurting me. I need air.' When the child's head came up off the floor, they slammed it back down," said Edith Ruland.

Three different state agencies are investigating the most recent death at Chad.

"I want justice," said Edith Ruland.

Officials at Chad have never talked to Channel 4, and on Friday representatives sent an employee outside to tell the station not to park on their street.
